One of the most popular plastic surgery procedures in the country, breast augmentation gives women with unevenly sized or small breasts a fuller, firmer and better proportioned look by using implants – made out of silicone or saline – and implanting them in the breast.

Patients may opt for the procedure for many reasons, like wanting to balance the breast size, post-pregnancy breast enhancement, correcting a congenital defect and many other reasons both medical and aesthetic. Customized For Each Patient

Since every woman is different, Dr. Gonzalez spends time before the surgery making sure that he understands what the patient hopes to gain from the procedure. He will then decide on the right implant type, size and placement. This is how every one of his patients leaves happy having received a customized treatment designed especially for her.

During their consultation, the patient will choose the breast implant size, placement and type. Since every human body is different, Dr. Gonzalez will make sure the patient is explained all the options available, based on her measurements, frame and expectations.

Complementary Surgeries

It is possible to achieve remarkable and beautiful results just through breast augmentation, but it can also be used along with other surgical or non-invasive procedures. Doctors sometimes recommend getting breast augmentation with another procedure, like a breast lift, for a better result. This is suggested mostly when treating breasts for sagging after pregnancy or during aging. In a combined procedure like this, some of the breast tissue is removed and the skin is tightened after which the implant is inserted.

Saline and Silicone Gel Breast Implants

Filled with saline or silicone gel, most breast implants are silicone shell type objects. Either of the fillings is safe to use but both have their own advantages. During the procedure, the implants are placed behind the breast; under either the tissue, or behind the chest wall muscles.

One of the advantages of saline breast implants is that if it ruptures the solution is absorbed by the body. One of its disadvantages is that the implants might not look as natural as silicone implants in patients with small breasts, or those who have had reconstructive surgery. This is something important that women who have little breast tissue to cover the implant with or those who have an injury or need breast reconstruction should keep in mind.

Silicone implants feel more natural, and are therefore the more popular implants. The U.S Food and Drug Administration has approved the use of silicone gel-filled implants after many years of testing. Dr. Gonzalez is therefore proud to be able to offer his patients these implants as an option since these are softer and feel more natural.

Incision Techniques Used in Breast Augmentation Surgery

Breast augmentation procedures are usually one to two hours long and are performed under general anesthesia. To minimize scar visibility, the incisions are made in inconspicuous places. The incisions may be located:

Armpit (transaxillary). With a transaxillary incision, the incision is made in the armpit tissue and the surgeon then creates a channel to the breast. Sometimes this procedure is done with an endoscope which is a small tube with a light and camera embedded in it, allowing the surgeon to see in the channel. The biggest advantage of this procedure is that the scar is not on the breast.

Crease under the breast (inframammary). This type is done with an incision made near the inframammary fold and under the breast. This is done both to reduce the visibility of the scar and to properly place the implant. This allows for the best  visibility since the surgeon is working right next to the breast.

Areola, which is the dark skin surrounding the nipple (periareolar). With this method, the incision is made on the boundary of the areas were the dark areola and remaining breast tissue meet. This allows for the implant to be placed precisely and also for better control over bleeding.

After the incision is made, the surgeon makes a pocket for the implant and then slides the implant into the incision and under the nipple.

It is also possible to place saline implants endoscopically via the navel. This in known as a transumbilical breast augmentation, or TUBA approach, and is the least invasive of all procedures

Placement Options During Breast Augmentation Surgery

Placing the implant under the chest muscle is preferable to placing it under the breast tissue only. This is because the risk of tightening of the skin around the implant (capsular contracture) is lowered and there is less interference during mammogram examinations. Disadvantages of placing the implant under the muscle include the need for drainage tubes and a higher level of pain for the first few days post surgery. Dr. Gonzalez can, however, help you decide which option would be better for you.

The implants cannot be placed under the chest wall in the TUBA procedure.

After inserting and positioning the implants under the nipples, the incisions are closed and then bandaged. After a few days the bandaging can be removed and a surgical bra can be used in its place.

Shape and Profile of Breast Implants

When picking implants the shape is something that must be considered. There are two basic shapes: round and teardrop. Since every woman’s breasts are different, the perfect breast implant must be chosen after considering both shapes and deciding based on the anatomy of the patient.

Round Implants. These are the most common type of shape used during breast augmentation procedures. Since they provide the most lift, fullness and cleavage, women prefer these over the other shape. However, some women feel that the shape they provide is more artificial, and therefore do not opt for these.

Around breast implants surface can either be smooth or textured. Since it is symmetrical in shape, there is no risk of the implant rotating.

These come in three variations in shape: low, medium and high profile. The higher profile implant will increase the forward projection of the breasts, which would otherwise only be possible to a larger breast implant that has a wider base.

Teardrop Implants

These are shaped like a teardrop, as the name suggests, and are contoured. They provide a gentler slope to the breasts. While the upper portion of a breast that has teardrop implants might not be as full, they provide more projection through volume. These are the ideal choice for women who are looking for a more natural breast shape.

These require a textured surface otherwise there is risk of rotation after the procedure which can distort the shape of the breasts, something that is not a concern with the round implants.
